Note when symptoms started, their severity (1–10), what makes them better or worse, and any medications you've tried. This helps your clinician diagnose more accurately and efficiently.
Include all prescriptions, over-the-counter medications, supplements, and vitamins — along with doses and how often you take them. This prevents dangerous drug interactions and saves time at your visit.
Blood pressure, resting heart rate, weight, and blood glucose (if applicable) are key numbers to monitor at home. Having a recent record helps establish trends that can reveal early warning signs.
Call 911 or go to your nearest ER immediately for chest pain, difficulty breathing, sudden severe headache, signs of stroke (face drooping, arm weakness, speech difficulty), or uncontrolled bleeding. Weekend NP is not an emergency service.
Weekend NP uses HIPAA-compliant systems to protect your health information. All telehealth visits occur over encrypted, secure video connections. Your records are never sold.
You have the right to request copies of your medical records. Contact Weekend NP through the patient portal to submit a records request. Responses are provided within 30 days.
